Qualifications

• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy Degree.
• Active Clinical Pharmacist license in the state of Illinois (IDFPR).
• Preferred: residency/training in specialty areas such as neonatology, critical care, oncology.

Responsibilities

• Interprets medication orders to ensure safety and proper use, coordinating with pharmacy, nursing and prescribers.
• Monitors therapy for drug interactions and therapeutic duplications, intervening as needed.
• Documents and communicates clinical interventions and maintains patient profiles and records.
