Qualifications

• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy (PharmD)
• Active Clinical Pharmacist license in Illinois (IDFPR)
• Willingness to float to multiple hospitals as needed
• Strong communication with physicians, nurses and patients

Responsibilities

• Interpret medication orders for safety and proper use.
• Monitor therapy and intervene on significant interactions or duplications.
• Document clinical activities in the EHR.
• Educate patients and staff about medications.
• Participate in medication-use review and quality improvement activities.
