Job summary

Northwestern Medicine is seeking a Clinical Pharmacist in Illinois to support patient care across the hospital's pharmacy services. The position is 30 hours per week on a rotating schedule, offering comprehensive benefits and a pathway to professional development.

The Clinical Pharmacist will identify, prevent, and resolve medication-related problems, provide information to health care professionals and patients, and ensure accurate documentation and integration with the electronic health record

Qualifications

• Active Clinical Pharmacist license in the state of Illinois (IDFPR).
• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy Degree.

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications and coordinates problem solving between pharmacy, nursing and prescribers.
• Monitors and intervenes in significant medication interactions and therapeutic duplications.
• Maintains and documents comprehensive patient profiles and ensures accurate EHR entries.
• Compounds and dispenses pharmaceuticals including sterile products and chemotherapy.
• Educates patients and healthcare professionals about medications and participates in quality improvement activities.
