Job summary

Northwestern Medicine in Chicago is seeking a Clinical Pharmacist to join our Pharmacy Department. You will identify, prevent, and resolve medication related problems, provide authoritative information to clinicians and patients, and ensure that pharmacy services integrate with patient care.

You will interpret orders, monitor therapy, consult on pharmacokinetics, document interventions, and participate in quality improvement and education programs.

Qualifications

• Active Clinical Pharmacist license in Illinois (IDFPR) required.
• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy Degree required.
• Experience in hospital pharmacy settings preferred.

Responsibilities

• Interprets medication orders to ensure safety and proper use and coordinates problem resolution.
• Monitors therapy for drug interactions and intervenes when needed.
• Evaluates medication appropriateness and communicates issues to prescribers.
• Acts as consultant on pharmacokinetics and adverse reactions.
• Documents clinical activities and interventions in the Electronic Health Record.
