Job summary

Northwestern Medicine is hiring a Clinical Pharmacist to identify, prevent, and resolve medication-related problems and provide authoritative information to healthcare professionals, patients, and the public.

The Pharmacist will participate in therapeutic drug monitoring, document clinical activities, and ensure drug distribution processes align with policies and standards across the Northwestern Medicine enterprise.

Qualifications

• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy.
• Active Clinical Pharmacist license in Illinois (IDFPR).
• Preferred: residency program or equivalent hospital pharmacy experience.

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications; coordinates resolution of medication-related problems between pharmacy, nursing, and prescribers.
• Monitors therapy for drug interactions and counsels on potential issues.
• Prepares and dispenses medications including sterile products and chemotherapy.
• Maintains patient medication profiles and ensures accurate documentation in EHR.
• Ensures compliance with formulary and participates in quality improvement activities.
