Job summary

Northwestern Medicine is seeking a Clinical Pharmacist to join our Pharmacy team in Illinois. The role focuses on ensuring safe, effective medication use and collaborating with physicians, nurses and other healthcare professionals to optimize patient care.

You will interpret orders, monitor drug interactions, contribute to pharmacokinetic evaluations, and educate staff and patients. This is a hospital-based, on-site position with comprehensive benefits.

Qualifications

• Active Clinical Pharmacist license in the state of Illinois (IDFPR).
• Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree.

Responsibilities

• Interprets medication orders to ensure safety and proper use of medications.
• Monitors medication therapy for significant drug interactions and therapeutic duplications.
• Evaluates and documents appropriate medications for therapeutic outcomes.
• Communicates potential problems and solutions to prescribers.
• Processes medication orders into the Electronic Health Record and maintains patient profiles.
