Job summary

The Pennant Group, Inc. is seeking a Clinical Manager to coordinate and supervise clinical teams for home health and hospice operations in Fort Worth, TX.

You will oversee nurses, aides, social workers, and chaplains, and work with physicians to develop care plans and ensure on-site and on-call coverage. Ideal candidates hold an RN license (BSN preferred) with hospice/home care experience and prior management or supervisory exposure, driving continuous improvement and ensuring high quality

Qualifications

• Graduate of National League of Nursing accredited school of nursing with a BSN degree is preferred.
• Registered nurse with current licensure to practice professional nursing in the state.
• Complies with accepted professional standards and practice.
• CPR certification is maintained as required.

Responsibilities

• Coordinate and manage day-to-day clinical needs in the agency.
• Provide oversight of nurses, aides, social workers, and chaplains.
• Develop and update the individualized plan of care with physicians.
• Ensure on-call coverage and timely case reviews.
• Participate in quarterly record reviews and quality improvement.
