Job summary

The Pennant Group is seeking a Clinical Manager to coordinate patient care, supervise clinical staff, and ensure high-quality home health and hospice services in Idaho.

This RN-led role requires licensure in Idaho and Washington, plus 3 years in home care or hospice and 2 years in management. Responsibilities include staff education, care plan development, case conferences, and infection-control oversight.

Qualifications

• Must be a registered nurse with current licensure to practice in Idaho and Washington.
• Three years of previous home care or hospice experience and two years of management/supervision experience preferred.
• HCHB/PointCare experience preferred.
• Interdisciplinary collaboration and strong communication are essential.

Responsibilities

• Coordinate and manage day-to-day clinical operations and patient care.
• Educate and orient new clinical staff and oversee ongoing orientation.
• Supervise nurses, aides, social workers, and chaplains in field work.
• Develop, implement and update individualized care plans and communicate with physicians.
• Ensure on-call coverage, review referrals, and oversee case conferences.
• Participate in quality improvement activities and infection control.
