Qualifications

• Active RN license in the state.
• BSN preferred or 1 year RN experience within last 3 years.
• Hospice/home care background; 2+ years management/supervisory experience.
• CPR certification current; understands hospice care principles.
• HCHB and PointCare experience preferred.

Responsibilities

• Coordinate patient care and supervise clinical staff.
• Review referrals and assign hospice personnel; confer with Medical Director.
• Oversee medical records management in EMR system.
• Lead case conferences and quality improvement activities.
• Participate in hiring and orientation of staff.
