Job summary

Qualifications

• Must be a registered nurse or licensed clinician with current licensure to practice in the state.
• Experience in home care setting preferred and supervisory experience preferred.
• Excellent observation, nursing judgment and communication skills.

Responsibilities

• Coordinate and manage patient care with clinicians and staff.
• Ensure timely documentation and attainment of productive expectations.
• Develop and update individualized plans of care in collaboration with physicians.
• Lead case conferences and coordinate care with organization personnel.
• Participate in quarterly record reviews and QA programs.
