Job summary
The Pennant Group, Inc. is seeking an experienced Home Health Nurse Supervisor to lead interdisciplinary care planning in the Fort Worth area.
You will develop and initiate individualized plans of care with patients, families, and caregivers, performing preventative and rehabilitative nursing procedures as ordered by physicians. Qualifications
- RN license in the state where practicing.
- CPR certification current and valid.
- Minimum two years of experience, public health or home care preferred.
- Self-directed with minimal supervision and strong communication skills.
Responsibilities
- Develops and initiates plan of care with interdisciplinary team and patient/family.
- Carries out preventative and rehabilitative nursing procedures as ordered.
- Provide counseling and education to patient, caregiver, and family.
- Prepare clinical notes and communicate with physician and other care professionals.
- Coordinate discharge planning and ensure post-discharge needs.
- Supervise home health aides and licensed staff; on-call duties as policy.
