• Current RN license to practice in the state.
• CPR certification is required.

Responsibilities

• Complete initial, comprehensive and ongoing patient/family assessments for hospice needs.
• Develop and implement care plans with patient and family involvement.
• Administer medications and treatments as prescribed by the physician.
• Coordinate patient care as Case Manager with the interdisciplinary team.
