• Clinical work experience

Responsibilities

• Direct Patient Care: Intervene as appropriate to improve prescribing, administration, and monitoring practices within the facility.
• Medication Therapy Management (MTM): Provide MTM services and patient education as defined in the Illinois Pharmacy Practice Act.
• Dosing Recommendations: Determine optimal medication doses based on patient age, weight, and renal/hepatic function.
• Collaborate with hematology/oncology and inpatient pharmacy team, with both inpatient and outpatient responsibilities.
• Facilitate safe and timely preparation of investigational and standard of care medications for clinical trials.
• Attend hematology/oncology rounds and provide pharmacy services in the interdisciplinary practice at Carle Foundation Hospital.
• Collaborate with hematology/oncology providers to enhance patient-centered care.
• Provide safe and effective drug therapy management to patients.
• Review, monitor and manage adverse effects.
• Monitor care plan for regimen-specific adherence and toxicities to patient’s oral chemotherapy regimen.
• Oversee and direct the preparation of chemotherapy.
• Assist with transitions of care, support clinical trial protocols, staff development, and precept hematology/oncology students and residents.
• Participate in treatment plan and therapy plan development and collaborate with committees for implementation.
• Develop medication-related policies, procedures and protocols, and promote safety initiatives for oncology patients.
• Participate in pharmacokinetics dosing service activities, hospital committees and other duties as assigned
