Carle Health in Illinois is seeking a licensed pharmacist to deliver comprehensive oncology pharmaceutical care within the Carle Cancer Center, collaborating with hematology/oncology, urology, ophthalmology, Gynecology, Research, and Expanded Care.

In this role you will provide MTM, dosing recommendations, chemotherapy preparation, monitor adverse effects, and participate in rounds and clinical trials to advance patient-centered care.

Qualifications

Licensed Pharmacist in Illinois (IDFPR)
Bachelor’s Degree in Pharmacy required
Clinical work experience required

Responsibilities

Direct patient care to optimize prescribing, administration and monitoring.
Provide MTM services and patient education per Illinois Pharmacy Act.
Determine dosing based on age, weight, renal/hepatic function.
Collaborate with hematology/oncology and inpatient pharmacy teams.
Prepare chemotherapy and investigational medications for trials.
Attend rounds and provide pharmacy services in interdisciplinary teams.
Participate in treatment plan development and safety initiatives.
Develop oncology-related policies and safety procedures.
