Responsibilities

• Direct patient care: intervene to improve prescribing, administration, and monitoring practices.
• Medication Therapy Management (MTM): provide MTM services and patient education.
• Dosing recommendations based on age, weight, and organ function.
• Collaborate with hematology/oncology and inpatient pharmacy teams, inpatient and outpatient.
• Oversee preparation of chemotherapy and investigational medications for trials.
• Attend rounds and provide pharmacy services in hematology/oncology.
• Develop medication-related policies and safety initiatives.
• Participate in pharmacokinetics dosing service activities and committees.
