Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Clinical Navigator to join our team as a full-time professional. You will meet patients at their toughest times and drive the referral and admission process within our LTACH setting, helping families understand goals and available care options.

Collaborating with admissions, nursing, and other staff, you will cultivate relationships with physicians and payers, ensuring high-quality patient transitions and satisfaction.

Qualifications

• Education and Training: Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts and analyzes referral and admission data from assigned territory.
• Works collaboratively with Admissions Manager and admission staff, nursing and other internal and external staff to facilitate the referral conversion.

Current licensure in nursing, RT, PT, OT, or SLP
