Job summary

Join PAM Health Specialty and Rehabilitation Hospital of Covington as a Clinical Navigator. You will be the face of the hospital, meeting patients at difficult times and outlining how rehab can help them achieve their goals.

You will build and maintain referral relationships, assess patients, and work with Admissions and nursing teams to move referrals forward. The role emphasizes collaboration, program feedback, and ensuring high satisfaction of patients, families, physicians, discharge

Qualifications

• Minimum two years’ experience in the designated field (rehab/LTACH).
• Degree in a health-related field from an accredited college or university; current licensure required.
• Prior marketing and/or rehab/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Analyzes referral and admission data from the territory.
• Collaborates with Admissions Manager, admission staff, nursing and other teams to facilitate referrals.
• Provides feedback for program development and quality improvement in customer service and referrals.

Education

Degree in a health-related field

Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy
