Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Clinical Navigator to cultivate referral relationships, manage a defined territory, and assess patients during rehab stays. You will collaborate with admissions, nursing, and support staff to optimize referrals and admissions.

The role requires a health-related degree and active licensure, with at least two years in the relevant field, preferably rehab/LTACH and marketing experience.

Qualifications

• Education in health-related field and active licensure required.
• Minimum two years of experience in the licensed field, preferably rehab/LTACH and marketing.
• Experience in building referral networks and patient/physician relations is preferred.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and conducts sales contacts.
• Analyzes referral and admission data with admissions staff to optimize conversions.
• Provides feedback for program development and quality improvement related to customer service and referrals.
• Maintains relationships with patients, families, physicians, nurses, discharge planners, and payer representatives.
