Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Clinical Navigator to cultivate referral relationships, manage an assigned territory, and complete patient assessments within a collaborative hospital setting.

You will work with the Admissions Manager, nursing staff, and external partners to optimize referrals and admissions, while contributing to program development and service quality improvement efforts.

Qualifications

• Education and Training: Degree in a health-related field from an accredited college or university preferred.
• Experience: Must have a minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments. Makes sales contacts as well as analysis of referral and admission data from assigned territory. Works collaboratively with Admissions Manager and staff to facilitate referral conversion and patient satisfaction.
