Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Clinical Navigator to be the face of our hospital, building referral relationships and coordinating patient admissions during rehab stays. The full-time role collaborates with the admissions team and nursing staff to ensure smooth referral and admission processes while supporting patients and families through the recovery journey.

Requires health-related degree and applicable licensure with rehab/LTACH experience

Qualifications

• Education and Training: Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts as well as analysis of referral and admission data from assigned territory.
• Works collaboratively with admissions staff, nursing and other internal and external staff to facilitate referral conversion.
• Provides feedback and recommendations for program development and quality improvement initiatives related to customer service, the referral/admission process and patient/referral source satisfaction.
• Develops and maintains excellent relationships with stakeholders including prospective patients, families, physicians, ICU/floor nurses, discharge planners, case managers and payer representatives.

Education

Degree in a health-related field

Current licensure in nursing / therapy (any listed field)
