Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Clinical Navigator to serve as the face of our hospital. You will cultivate referral relationships, manage an assigned territory, and complete patient assessments to support admission decisions.

Under the direction of the Director of Business Development, you will collaborate with admissions staff, nursing, and other teams to optimize the referral process and patient satisfaction.

Qualifications

• Education and Training: degree in a health-related field; licensure required.
• Must have a minimum of two years' experience in the designated field of license.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and analyzes referral/admission data.
• Works with admissions, nursing, and other staff to facilitate referral conversion.
• Develops relationships with patients, families, physicians, discharge planners, and payer representatives.

Education

Degree in a health-related field from an accredited college or university

Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy
