Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Clinical Navigator to engage patients at the point of contact, cultivate referral relationships, and manage a defined geographic territory. You will coordinate with admissions and nursing teams to streamline referrals, admissions, and post-acute transitions.

Qualifications

• Degree in a health-related field from an accredited college or university is preferred.
• Current licensure in nursing, respiratory, physical, occupational, or speech therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Performs patient assessments and facilitates referral and admission processes.
• Collaborates with admissions and nursing staff to improve customer service and satisfaction.
• Provides feedback for program development and quality improvement initiatives.
