Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington seeks a Clinical Navigator to lead referral development and patient admissions within a designated territory. You will build relationships with physicians, discharge planners, and payers, conduct patient assessments, and work with admissions staff to optimize the referral-to-admission process.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts and analyzes referral and admission data from the territory.
• Collaborates with Admissions Manager and staff to facilitate referral conversion.
• Provides feedback for program development and quality improvement in customer service and the referral/admission process.
• Maintains relationships with stakeholders including patients, families, physicians, ICU/floor nurses, discharge planners, case managers, and payers.
