PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Clinical Navigator to cultivate referral relationships, manage an assigned territory, and complete patient assessments. The role involves making sales contacts and analyzing referral and admission data to drive successful conversions.

You will collaborate with the Admissions Manager, nursing staff, and external partners to optimize the referral process.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.
• Minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.

Responsabilités

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts as well as analysis of referral and admission data from assigned territory.
• Works collaboratively with the Admissions Manager and admission staff, as well as nursing and other internal and external staff to facilitate referral conversion.
• Develops and maintains relationships with stakeholders including patients, families, physicians, discharge planners, case managers and payer representatives.
