Job summary

PAM Health in Humble, TX seeks a dedicated Clinical Navigator to be the face of the hospital, cultivating referral relationships and driving patient admissions. You will meet patients at difficult moments, share potential outcomes, and support rehab goals with a collaborative team including admissions staff and nurses.

The role requires a health-related degree, current licensure in a relevant field, and at least two years of experience in rehabilitation/ LTACH or marketing.

Qualifications

• Degree in health-related field preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech-language pathology is required.
• Must have a minimum of two years’ experience in the designated field of license, preferably with rehabilitation/ LTACH and marketing experience.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts and analyzes referral/admission data with the team.
• Develops and maintains relationships with stakeholders including patients, families, physicians, discharge planners, case managers and payer representatives.
