Oak Street Health is seeking a Medical Scribe to support primary care providers with clinical documentation. You will observe and document in real time, join the exam room visits, participate in morning huddles, and assist with next steps after visits.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing and/or volunteering, preferred.
• Scribe or transcription experience preferred but not required.

Responsibilities

• Document patient encounters in a structured note with history, assessment, plan, and exam.
• Assign appropriate CPT and ICD-10 codes.
• Prepare After Visit Summaries.
• Consult with provider on documentation opportunities and support care team.
