Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers by documenting patient encounters in real time. You will observe and document within the exam room, join daily huddles, and assist with next steps in the patient care journey.

Ideal candidates have strong medical terminology knowledge, clinical exposure, and the ability to use documentation tools efficiently. This is a full-time, on-site role in our clinics, with comprehensive benefits and ongoing training in

Qualifications

• Knowledge of medical terminology and common medications or prior clinical experience (required)
• Prior clinical experience including shadowing/volunteering (preferred)
• Prior scribe or transcription experience (optional)

Responsibilities

• Document patient encounters in a structured note with history, assessment, plan, and exam
• Assign CPT and ICD-10 codes for encounters
• Prepare After Visit Summaries
• Consult with provider to ensure accurate documentation
• Support clinical documentation improvements using population health tools
