Responsibilities

• Documenting patient encounters in a structured note with history, assessment, plan, and exam
• Assigning CPT and ICD-10 codes for visits
• Preparing After Visit Summaries and supporting documentation
• Collaborating with providers to ensure accurate and timely documentation
