Oak Street Health is seeking a Medical Scribe to support our primary care providers by documenting encounters in real time, coding with CPT and ICD-10, and preparing after-visit summaries.

The CIS/Scribe attends daily huddles, observes visits in the exam room, and helps close care gaps within our value‑based care model. This full-time role offers on‑the‑job training, exposure to clinical workflows, and a pathway toward health informatics or healthcare administration.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience preferred.
• Scribe or transcription experience preferred.

Responsibilities

• Document patient encounters in structured notes.
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries.
• Support Daily Huddles and CDS reviews.
