Job summary

Oak Street Health is seeking a Medical Scribe (CIS) to support primary care providers with clinical documentation, enabling clinicians to focus on delivering exceptional patient care. Scribes observe in exam rooms, document encounters in structured notes, assign CPT/ICD-10 codes, and prepare After Visit Summaries.

They become experts in value-based care documentation and patient care gaps, with on-the-job training provided.

Qualifications

• Knowledge of medical terminology and common medications, either from a pre-medical degree or prior clinical experience.
• Prior clinical experience, including shadowing and/or volunteering [strongly preferred].
• Prior scribe or transcription experience [preferred but not required].
• Advanced listening and communication skills [required].
• Strong computer literacy and ability to learn new technical workflows [required].

Responsibilities

• Documenting Patient Encounters ~ 80%: joining the provider in the exam room, documenting visits in structured notes, assigning CPT and ICD-10 codes, preparing After Visit Summaries, ensuring accurate documentation.
• Clinical Documentation Improvement ~ 10%: reviewing medical records, leveraging population health tools to support documentation improvement.
• Preparing for and supporting Daily Huddles and Clinical Documentation Reviews.
• Administrative support for the provider and care team: placing orders and referrals, addressing tasks, supporting documentation-related responsibilities.
