Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers with real-time documentation. You will observe visits, document encounters in structured notes, and help ensure accurate coding and care gaps closure.

Qualifications

• Knowledge of medical terminology and common medications (required).
• Advanced listening and communication skills (required).
• Strong computer literacy and ability to learn new workflows (required).

Responsibilities

• Document patient encounters in real time and create structured notes with history, assessment, plan, and exam.
• Assist with CPT and ICD-10 coding and prepare After Visit Summaries.
• Support clinical documentation improvement by reviewing records and leveraging population health tools.
• Provide administrative support to the provider and care team as needed.
