Qualifications

  • High School Diploma or equivalent with three (3) years of relevant work experience.
  • Two (2) years of experience working with children and/or adults with Mental Health diagnoses, Intellectual and Developmental Disabilities (IDD), and/or Drug & Alcohol treatment populations.
  • Valid Driver's License.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to collaborate effectively with multidisciplinary teams.

Responsibilities

  • Maintain up-to-date knowledge of RHD programs and ensure implementation of best practices.
  • Remain knowledgeable about program regulations, community support services, licensing requirements, and funding sources.
  • Build and strengthen relationships with program staff, healthcare providers, and community partners to coordinate care and services.
  • Maintain tracking systems to identify individuals requiring comprehensive treatment planning.
  • Conduct monthly and quarterly coordination visits with internal and external providers.
  • Provide direct supervision and support to Care Coordinators and Resource Coordinators.
  • Facilitate routine multidisciplinary treatment team meetings and collaborative planning sessions.
  • Monitor and report quality outcomes, statistics, and performance measures.
  • Complete weekly and monthly reporting requirements accurately and timely.
  • Conduct systematic reviews of client records to ensure comprehensive care coordination and resource utilization.
  • Develop comprehensive treatment plans for individuals receiving services across multiple programs and settings.
  • Review treatment plans to ensure compliance with program, funding, and regulatory requirements.
  • Collaborate with internal departments and external partners to promote quality care and positive outcomes.

What You'll Do
