Job summary

CVS Health is seeking a clinician who applies advanced clinical judgement to coordinate member physical and behavioral health care, direct provider collaboration, and psychosocial wraparound services. You will utilize available resources to achieve optimal, cost-effective outcomes.

Requirements include active licensure (LCSW/LPC/LMFT), case management experience, strong communication and motivational interviewing skills, and meticulous documentation.

Qualifications

• Current/Active Clinical Licensure (LCSW, LPC, LMFT) required.
• Case Management experience.
• Strong customer service orientation and problem solving.
• Excellent Communication/Telephonic skills.
• Excellent motivational interviewing skills and ability to build rapport telephonically.
• Excellence in documentation and regulatory compliance.
• Highly organized, able to work with minimal supervision.
• Ability to manage time effectively and meet performance metrics.
• Technology proficiency.
• Ability to travel within 125 miles round trip for 1 day in the field.
• Variable work schedule with ability to work 2 days weekly until 9pm local time.
