Fairview Health Services in Bloomington, MN is hiring an In Clinic Visit Facilitator for the Primary Care team. This 1.0 FTE role supports in-person visits, pre-visit communications, and documentation in patient records, ensuring exam rooms are stocked and tasks assist the team’s efficiency.

The role involves patient preparation, rooming assistance, and coordinating follow-up appointments, with a focus on a positive patient experience and smooth care delivery.

Qualifications

• At least 1 year of customer service experience.
• Proficient with technology (computers, devices).
• Current nursing, medical, social work, psychology, or pharmacy student (preferred).
• Successful completion of NA, EMT, or HUC program (preferred).
• Experience with medical terminology, Epic Health Records, and telephonic customer experience (preferred).

Responsibilities

• Prepare patients for their appointments, ensure records are collected and available.
• Assist in rooming and document patient information (reason for visit, meds, vitals, health questionnaires).
• Prepare exam rooms and stock supplies.
• Support patient connection to provider and ensure a positive patient experience.
• Handle visit wrap-up and follow-up scheduling.
• Communicate effectively with patients, providers, and health professionals in person and via telehealth.
