Job summary

Fairview Health Services in Minneapolis, MN is seeking an In Clinic Visit Facilitator to support Rheumatology, Nephrology and Infectious Disease care teams at our Riverside clinic. This casual/on-call, 0.01 FTE role involves assisting with patient check-ins, preparing exam rooms, collecting pre-visit information and guiding patients through the care experience.

The facilitator will help gather medical information, document in the electronic health record, stock rooms, coordinate follow-up

Qualifications

• 1+ year of customer service experience.
• Proficient with technology (hardware and software, smartphones, tablets).

Responsibilities

• Prepare patients for appointments and collect medical information prior to visits.
• Assist in rooming, document patient information and vitals.
• Prepare exam rooms and stock supplies.
• Support patient connection to providers and ensure a positive experience.
• Assist with scheduling and follow-up coordination as needed.
• Communicate effectively with patients, providers and health professionals.
