Job summary

Redwood Family Care Network is seeking an Client Relations & Intake Manager in Northern California to manage referrals, coordinate intake, and support client placement with a focus on person-centered service delivery.

You will maintain communication with referral sources, families, and program teams, track referrals, and help identify opportunities to improve access to services across the California IDD community.

Qualifications

• Bachelor's degree in related field preferred or equivalent experience.

Responsibilities

• Serve as primary contact for referrals, inquiries, and prospective clients.
• Maintain referral/intake tracker with source, needs, status, and follow-up.
• Coordinate intake with internal departments and leadership.
• Identify barriers to admission and resolve issues.
• Build relationships with Regional Centers, hospitals, and community partners.
