HealthRIGHT 360, North County Serenity House in California, seeks a Case Manager to coordinate care across outpatient, residential, and community settings, assist with intake, document assessments, and help clients apply for Medi-Cal and other benefits.

You will connect clients to mental and physical health services, housing, employment support, and probation and DCFS resources, while maintaining HIPAA-compliant records and meeting productivity expectations.

Qualifications

• High school diploma or equivalent.
• Substance Use Disorder Counselor registration from an accredited California agency (CCAPP, CAADE, CADTP).
• Minimum two years of experience as a job developer or employment specialist.
• Experience teaching adults and/or facilitating workshops.
• Demonstrated experience and commitment to working with diverse communities, including communities of color and LGBTQ+ individuals, those experiencing housing insecurity or homelessness, individuals with substance use challenges, people living with HIV/AIDS, and persons with mental health conditions.

Responsabilités

• Assist the client with intake by completing case management assessment and entering financial and benefit information.
• Support the client in applying for Medi-Cal or transferring Medi-Cal County when appropriate.
• Assess the client’s case management needs and complete all releases of information.
• Connect the client to benefits, mental health, physical health, employment, probation, DCFS, housing, community resources, outpatient substance use disorder services, and aftercare.
• Coordinate communication and external service linkage including assisting with scheduling appointments, communicating with probation, scheduling child visits, communicating with DCFS, obtaining all court minute orders, providing appointment reminders for therapy and psychiatrist.
