Job summary

The Columbus Organization is hiring an RN to support individuals with IDD across Northern California, primarily Chico and Redding. The role emphasizes clinical assessment, care coordination, and advocacy to ensure access to medical services and supports, in community settings.

Candidates should have CA RN licensure, a BSN preferred, and at least three years of nursing experience. Travel within the service area is required, and collaboration with multidisciplinary teams is essential.

Qualifications

• Active and unrestricted RN license in CA.
• BSN preferred.
• Minimum of three years of professional nursing experience.
• Strong clinical assessment and documentation skills.
• Ability to work independently and with interdisciplinary teams.

Responsibilities

• Conduct comprehensive nursing assessments across medical, environmental, psychosocial, educational, and self-care needs.
• Review and monitor medical and nursing status for community-based services.
• Occasional mortality review committee involvement.
• Identify needs for specialized medical services and referrals.
• Collaborate with physicians, families, and teams to support care planning.
• Serve as liaison with hospitals, SNFs, and community agencies.
• Participate in discharge planning and care coordination.
• Coordinate specialty consultations and clinical services as needed.
• Develop nursing care plans, clinical recommendations, and health protocols.
• Provide education to caregivers and providers on health needs and best practices.
• Assist with program eligibility reviews and regulatory documentation.
• Advocate for appropriate healthcare services.
• Maintain accurate clinical documentation and case records.
