Job summary

Northern California Behavioral Health System is seeking a Case Manager to collaborate with Inpatient Therapists delivering quality case management and discharge planning for patients and families. You will participate in an interdisciplinary team to support the treatment program and ensure linkage to mental health services, coordinating follow-up and placement logistics.

The role requires a Bachelor's degree in psychology or social work, 1 year of behavioral health experience, CPR and HWC

Qualifications

• Bachelor’s Degree in Psychology, Social Work, or closely related field.
• One year of behavioral health experience and discharge planning preferred.
• Current CPR certification or during orientation period.
• Current HWC certification or during orientation period.
• Familiarity with continuum of care and clinical terminology.

Responsibilities

• Provide case management and discharge planning services in collaboration with Inpatient Therapists.
• Serve as a member of the interdisciplinary team supporting the treatment program.
• Ensure linkage to mental health/recovery services and communication with County Liaison.
• Arrange follow-up appointments, referrals to pharmacy, and placement support.
• Complete required administrative documentation in the clinical record and related forms.
