Qualifications

• Bachelor's or Master's degree in Social Work or a related field.
• Strong written, verbal, and interpersonal communications skills.
• Good time management and organizational abilities.

Responsibilities

• Provide case management services and advocacy in the Intensive Case Management Unit.
• Maintain a caseload up to 1:25 and conduct monthly face-to-face monitoring visits.
• Complete required forms and reports; coordinate with community agencies and staff.
