3–5 years of client- or patient-facing experience in healthcare or social services.
Experience performing intakes in a healthcare environment.
Strong communication, listening, relationship-building, and problem-solving skills.
Strong organizational skills and ability to manage multiple client needs in a high-volume setting.
Experience with a CRM system such as Salesforce is preferred.

Responsibilities

Enroll eligible applicants into programs and engage with clinicians, social service providers, and community members.
Handle high-volume calls and emails, provide customer support, and maintain detailed data entry.
Serve as primary contact for assigned clients and connect them to internal/external resources.
Provide accurate information on services, eligibility, and program details to clients, clinicians, and referrers.
Maintain confidential documentation of client interactions and referrals.
