Job summary

The Guidance Center Inc. is seeking a Client Navigator to support clients in outpatient services. The role focuses on data collection, brief interviews when needed, and coordinating with care providers to optimize outcomes.

The ideal candidate is organized, professionally competent, and able to use a trauma-informed, person-centered approach to meet consumer needs and program requirements.

Qualifications

• Proven ability to collect and organize client information.
• Experience supporting outpatient service programs and outcomes.
• Familiarity with trauma-informed, person-centered approaches.

Responsibilities

• Meet with referred individuals to gather information and provide support.
• Advocate for consumers, linking and referring to needed services with follow-up.
• Provide referral information and communicate with treatment providers on outcomes.
• Perform clinical and administrative tasks per agency policies.
• Enter demographics and insurance data into electronic systems; maintain records.
