WCDC, Inc. is seeking a Service Coordinator to support individuals and families with coordinated, person-centered, and culturally responsive services. The role emphasizes stability, independence, and well-being through collaboration with internal and external partners.

The position requires strong organizational skills, a trauma-informed approach, and experience with client documentation and community resources. Pay is based on education and experience, with regular business hours.

Qualifications

• High School Diploma or equivalent required.
• Associate or bachelor’s degree in social work, human services, psychology, or related field preferred.
• Experience in direct support or nonprofit setting preferred.
• Familiarity with local community resources and public benefits preferred.

Responsibilities

• Ensure safety and person-centered support plans are followed.
• Conduct intake and assessments to determine client needs and goals.
• Develop individualized service plans and assist clients in achieving goals.
• Coordinate services and referrals with partners and internal programs.
• Provide follow-up, advocacy, and connection to resources.
• Maintain confidential client documentation per requirements.
• Collaborate with team to promote communication and outcomes.
• Participate in team meetings and trainings.
• Supervise practicum students and provide feedback as needed.
• Identify opportunities for service expansion such as residential or supported living programs.
