The Guidance Center Inc. is seeking a Client Navigator to support clients in outpatient services. The role focuses on data collection, brief interviews when needed, and coordinating with care providers to optimize outcomes.

Job description

Job Overview: The Client Navigator will provide a range of clinical and administrative support services to clients referred for outpatient services. This individual is expected to assist clients entering the agency by managing data collection and follow up, conducting brief interview when required, and coordinating with internal/external care providers to optimize service outcomes. The Client Navigator promotes agency-wide focus on results, service quality, and customer satisfaction. The ideal candidate will be highly motivated, well-organized, professionally competent, and capable of using a trauma-informed, person- centered approach to meet individual consumer needs and program requirements

Responsibilities and Duties:

• Meet with individuals referred for outpatient services to gather necessary information and provide support.
• Maintain up-to-date understanding of programs and resources available for consumers who need additional support; Advocate for consumers and provide linkages, referrals, and follow-up for services/supports based on needs identified during any consumer interaction.
• Provide relevant referral information for clients when indicated and communicates with treatment providers regarding outcomes of assessments and interviews when needed.
• Performs relevant clinical and administrative tasks according to agency policies.
• Enters required data (demographics, insurance, etc.) into identified electronic systems, maintains records, and monitors timelines.

Education and Experience:

• High School graduate or equivalent with relevant experience working with individuals; LPN or individual with Bachelor’s Degree in Psychology, Community Health or related field preferred.
