Job summary

Child & Family Services, Inc. seeks a Recovery Support Navigator to assist individuals on their recovery journey. You will conduct screenings, intakes, and referrals, collaborating with recovery coaches to coordinate services and supports.

You will provide trauma-informed, recovery-oriented, person-centered care, help consumers clarify goals, and access community resources, including transportation and after-hours care when needed.

Qualifications

• Bachelor’s degree in social work, psychology or related field required.
• Minimum two years of related work experience in addiction recovery or behavioral health.
• Excellent communication and strong organizational skills, detail-oriented and able to multitask.

Responsibilities

• Screening and intake interviews to determine consumer eligibility for services.
• Provide trauma-informed, recovery-oriented, person-centered care tailored to consumer needs.
• Explore treatment options, clarify goals, provide education and resources, and assist accessing services.
• Collaborate with care team to establish goals and support the consumer's care plan.
• Assist with transportation to medical/behavioral health appointments during transition to community resources.
• Provide RSN services seven days a week, including on-call and after-hours via telephonic coverage.
• Deliver services on a mobile basis in safe environments and maintain community resources connections.
• Maintain confidentiality and comply with 42-CFR and privacy laws.
• Document and maintain records per agency and funding source requirements.
