Job summary

Full Life Care in Bellevue, WA is seeking a Client Intake Supervisor for our Bellevue location. You will coordinate referrals, complete initial assessments, onboarding, and schedule ongoing in-home care services, transferring clients to a Home Care Supervisor as needed.

The role requires a AA degree or relevant experience, at least 1 year of supervisory experience, and strong English communication. A valid driver's license and background checks are required.

Qualifications

• Associate degree or relevant experience in social services or related field.
• At least one year of supervisory experience.
• Experience in health or social service setting preferred.
• Excellent written and verbal communication in English.
• Valid driver's license and automobile insurance verification.
• Background check and drug screen completed; other screenings may apply.

Responsibilities

• Coordinate, handle, and request new client referrals from local/state authorizers; respond to inquiries.
• Complete new client intake with initial assessments and onboarding.
• Schedule ongoing in-home care services and transfer clients to Home Care Supervisor.
• Supervise delivery of home care services; provide feedback and participate in improvement plans.
• Collaborate with leadership to meet program goals and support staffing efforts.
• Participate in recruiting events, interviews, and new hire orientation when applicable.
• Answer telephone calls and emails; perform other duties as assigned.
