Job summary

Transforming Age in Bellevue is hiring a Client Intake Supervisor to coordinate referrals, assessments, and scheduling for in-home caregiving services. You will supervise staff, ensure smooth client onboarding, and collaborate with leadership to meet program goals.

The ideal candidate has an AA degree or relevant experience, at least one year of supervisory experience, and strong English communication. A valid driver’s license and background checks are required, with health/service settings

Qualifications

• AA degree in social services or related field or relevant experience
• Minimum of one year of supervisory experience
• Experience in health or social service setting preferred
• Excellent written and verbal communication in English
• Valid driver’s license and automobile insurance verification
• Background check, drug screen and other pre-employment screenings may apply

Responsibilities

• Coordinate and request new client referrals from local/state authorizers; respond to prospective inquiries
• Complete new client initial assessment and onboarding, including screenings and in-person visits
• Schedule ongoing in-home caregiving services and transfer to Home Care Supervisor as needed
• Supervise delivery of home care services and participate in performance improvement as necessary
• Collaborate with leadership to meet program goals and support caregiver staffing
• Participate in Home Care Aide recruiting events and orientation when applicable
• Answer phone calls and emails as needed
• Perform other duties as assigned
