Home Instead Northampton is seeking a Client Care Coordinator to perform care consultations with potential clients and family members, and to create tailored service plans and ongoing follow-up to ensure high-quality care and client retention.

The role involves introducing clients to CAREGivers, coordinating care with the team, maintaining records of quality assurance meetings, and pursuing adjustments to service plans as needed.

Competenze

• College degree preferred but not mandatory.
• At least 1 year experience in home care or senior-healthcare industry.
• Must possess a valid driver’s license.
• Available to work evenings or weekends as needed.

Mansioni

• Meet with potential clients and families to discuss needs and create service plans.
• Conduct service inquiries and care consultations per consultative sales process.
• Maintain ongoing communication and follow-up with potential clients and families.
• Coordinate client care with team members and CAREGivers.
• Introduce clients and CAREGivers to new clients.
• Plan schedules to ensure monthly QA visits.
