Qualifications

• At least one year of experience in home care, health care, or a senior-related field.
• College degree preferred.
• Nursing, social work or direct care experience is valuable.
• Valid driver’s license required.

Responsibilities

• Meet with prospective clients and families to assess needs and plan a personalized service.
• Conduct care consultations and introduce clients and Care Pros.
• Perform quality assurance visits and update service plans.
• Coordinate with office staff, case managers, and partners to support client care.
• Provide ongoing training and coaching for Care Pros.
• Maintain accurate client records and documentation.
