Responsibilities include in-home consultations, care plan development, quality assurance visits, and collaboration with caregivers, healthcare partners, and staff. A Bachelor’s degree is preferred and strong communication, organization, and problem-solving skills

Responsibilities

• Serve as the primary contact for clients and their families.
• Build lasting relationships through ongoing communication and support.
• Advocate for high-quality care and service.
• Conduct in-home care consultations with prospective clients and families.
• Assess care needs and develop customized service plans.
• Follow up with prospective clients and referral sources regarding home care services.
• Conduct client and Care Professional introductions.
• Perform regular quality assurance visits and client satisfaction reviews.
• Monitor care plans and make recommendations as needs change.
• Ensure exceptional service delivery and client retention.
• Respond to client and family concerns with professionalism and urgency.
• Identify opportunities to enhance services and improve client outcomes.
• Collaborate with caregivers, families, healthcare partners, and office staff to ensure successful care experiences.
• Represent Home Instead at networking events and community programs.
• Build and maintain relationships with referral partners and community organizations.
• Support local marketing initiatives and community engagement efforts.
• Assist with presentations, sponsorships, and outreach activities to increase awareness of services.
• Maintain accurate client records, assessments, care plans, and QA documentation.
• Ensure compliance with company policies and procedures.
• Complete documentation accurately and timely.
